Drug Description

Indications 

Proctofoam HC is a prescription medication indicated for the treatment of minor pain, itching, swelling, and discomfort associated with hemorrhoids and other conditions affecting the anal area, such as anal fissures (tears in the lining of the anus or anal canal). 

Proctofoam HC combines hydrocortisone and pramoxine as its active ingredients. Hydrocortisone, a corticosteroid, exerts anti-inflammatory effects, reducing redness, itching, and swelling in the affected area. On the other hand, pramoxine, an anesthetic, provides temporary numbing relief to the treated area. 

Uses and Dosage 

Before using Proctofoam HC and with each prescription refill, carefully review its medication guide and patient information leaflet. Seek clarification from your doctor or pharmacist if you have any uncertainties. 

Proctofoam HC is for rectal use only and should not be taken orally. 

Typically, this medication is applied to the affected area 3 to 4 times daily as prescribed by the doctor. 

For anal administration, the provided applicator must be used. Follow these steps for proper application: 

  1. If possible, cleanse the affected area with mild soap and water, rinse thoroughly, and pat dry. 
  2. Shake the foam canister well before use. 
  3. Fill the applicator with the foam and hold it by the barrel. 
  4. Carefully insert the applicator tip just 1/2 to 1 inch (1.5 to 2.5 centimeters) inside the anus. 
  5. Push the plunger to release the foam, then carefully withdraw the applicator. 
  6. Disassemble the applicator and clean it thoroughly with warm water. 
  7. Avoid inserting any part of the foam canister into the anus, and do not insert the applicator more than 1/2 to 1 inch (1.5 to 2.5 centimeters) inside the anus. Refrain from using your finger or tissue to apply the product inside the anus. 

For perianal use, follow these steps: 

  1. Apply a small quantity of the medication to a tissue and gently massage it into the area. 
  2. Unless directed by your doctor, do not cover the treated area with plastic or waterproof bandages. Such coverings may increase medication absorption through the skin, potentially enhancing effectiveness but also raising the risk of side effects. 

Wash your hands immediately after applying Proctofam HC and avoid contact with your eyes, nose, ears, or mouth. If the medication comes into contact with these areas, promptly rinse them with clean water. 

For optimal results, use this medication exactly as directed and administer it consistently at the same times daily. Refrain from exceeding the prescribed dosage, frequency, or duration of use without your doctor’s approval. 

If you have been using Proctofoam HC for an extended period or in high doses, do not discontinue usage abruptly without consulting your doctor, for it may worsen certain conditions. Your doctor may need to reduce your dosage gradually. 

Notify your doctor if there is no improvement or if your condition worsens within 2 to 3 weeks. 

Side Effects 

Listed below are the potential adverse effects of Proctofoam HC. If any of them last or worsen, notify your doctor right away.

  • Dry skin 
  • Burning 
  • Itching 

Serious Side Effects 

  • Signs of weak adrenal gland: 
    • Severe upset stomach or vomiting  
    • Severe dizziness or fainting 
    • Muscle weakness 
    • Extreme fatigue 
    • Mood changes 
    • Loss of appetite or weight loss 
  • Signs of Cushing’s disease: 
    • Weight gain in the upper back or abdomen 
    • Moon face (facial swelling) 
    • Severe headaches 
    • Slow wound healing 
  • Signs of high blood sugar: 
    • Confusion 
    • Excessive sleepiness 
    • Increased thirst and hunger 
    • Frequent urination 
    • Flushing 
    • Rapid breathing 
    • Breath odor resembling fruit 
  • Irritation at the application site of the foam 
  • Severe rectal bleeding or rectal pain 

Contact your doctor right away and ask for medical help if you encounter any severe reactions. 

Allergic Reactions 

Although rare, a severe allergic reaction to Proctofam HC may occur. Immediate medical help must be sought if any of the following signs of severe allergic reactions are noticed:  

  • Rash 
  • Hives 
  • Itching 
  • Red, swollen, blistered, or peeling skin (with or without fever) 
  • Wheezing 
  • Tightness in the chest or throat 
  • Difficulty breathing, swallowing, or speaking 
  • Unusual hoarseness 
  • Swelling of the mouth, face, lips, tongue, or throat 

Not all side effects of Proctofoam HC are included in this list. Inform your doctor if any unlisted effects are observed. 

Cautions 

Before starting treatment with Proctofoam HC, inform your doctor if you have any known allergies, particularly to hydrocortisone, pramoxine, and other medications. 

Ask your pharmacist for more information, as Proctofoam HC may contain inactive components that may cause allergic reactions or other complications. 

To ensure Proctofoam HC is safe for you, disclose to your physician your medical background, especially if you have a history of: 

  • Diabetes 
  • Eye problems 
  • Infections (particularly rectal sores or infections) 
  • Liver disease 
  • Stomach or intestinal problems 

Ensure all your healthcare providers are informed of any current, newly started, or stopped medications, including prescriptions, over-the-counter drugs, vitamins, and herbal supplements, to prevent any drug interactions with Proctofoam HC. 

The use of Proctofoam HC in pediatric patients should be limited to the minimum effective dosage and duration. Prolonged corticosteroid therapy may potentially affect the growth and development of pediatric patients. 

Proctofoam HC should only be used during pregnancy when necessary, and excessive or prolonged use should be avoided. Consult your doctor to weigh the risks and benefits of this medication. 

The transfer of hydrocortisone or pramoxine in breast milk is uncertain. Consult your doctor before breastfeeding when using this medication.
