Uses and Dosage 

NeoMercazole is intended for oral administration exclusively. Initiation should be contingent upon the confirmation of hyperthyroidism through laboratory assessments. 

The initial dosage typically falls within the 20-60 mg range for adults. It should be adjusted in response to thyroid function measurements until the patient achieves a euthyroid state, thereby mitigating the potential for over-treatment and subsequent hypothyroidism. Subsequent therapeutic approaches may be pursued in one of two manners: 

  • Maintenance Regimen: The final prescribed dose generally falls within 5-15 mg daily and may be taken as a daily dose. This therapeutic regimen should be sustained for at least six months and up to eighteen months. Monitor thyroid function serially and make appropriate dosage adjustments to maintain a euthyroid condition. 
  • Blocking-Replacement Regimen: The dosage is retained at the initial level, i.e., 20-60 mg per day, concurrently with administering supplemental l-thyroxine at a dose of 50-150 mcg per day. This combined approach serves to prevent hypothyroidism. Therapy should continue for at least six months and up to eighteen months. 

Regarding the elderly population (65 or older), while there is no specific dosage regimen, exercise caution and adhere to contraindications and warnings due to reports indicating a potentially higher risk of a fatal outcome related to neutrophil dyscrasia. 

For children, the typical initial daily dosage is 15 mg per day. 

Side Effects 

Adverse reactions manifest predominantly within the initial eight weeks of the treatment course. Among the most commonly reported adverse effects are:

  • Arthralgia 
  • Mild gastric discomfort 
  • Skin rashes 
  • Pruritus
